What is the Foundations for Aged Care™ Program?

 Foundations for Aged Care™ is a structured, soft-handed personal development framework delivered within existing aged care support sessions, empowering workers to move beyond task-based care into meaningful, person-led outcomes.


It supports older Australians to:

  • clarify what matters most to them now 
  • express their wants, needs, and preferences clearly 
  • identify barriers impacting independence and wellbeing 
  • strengthen communication with carers, family, and providers 
  • build confidence in self-advocacy 
  • set realistic, meaningful goals 
  • take small, achievable steps toward improved quality of life 


This program shifts care from doing for to working with, ensuring the person remains active in shaping their own life.
